RN Case Manager
The Registered Nurse (RN) Case Manager will coordinate and oversee your care, tailoring services to meet your current and evolving needs throughout the course of your care. Responsibilities include comprehensive assessments, monitoring vital signs, providing wound care, offering education and support, and ensuring that all aspects of your care are effectively managed.
Social Worker
The hospice Social Worker provides support for emotional, financial, and practical needs. Assistance may include guidance with financial planning, accessing community resources such as Meals on Wheels, and connecting with other supportive services. The Social Worker is experienced in navigating complex family dynamics, particularly during end-of life care, and is committed to helping you and your loved ones develop a patient-centered care plan that aligns with your goals and values.
Chaplain
Chaplain visits are guided by your individual beliefs, values, and preferences. The hospice chaplain is trained to provide compassionate, inclusive support across all faiths, spiritual backgrounds, and cultures. Whether you seek religious guidance, spiritual support, or simply a comforting presence, the chaplain’s care is centered entirely around your needs. If you prefer not to discuss religion, visits can still offer meaningful support tailored to you.
Hospice Aide
The Hospice Aide will assist with personal care needs such as bathing, showering, or bed baths, based on what is safest and most appropriate as determined by the Registered Nurse and Power of Attorney (POA). Our aides are highly respectful of personal dignity and are committed to ensuring your comfort during each visit. Services may include the use of a towel warmer, aromatherapy, lotion therapy, music therapy, and other personalized services. Visits are scheduled several days a week, depending on your needs. Hospice Aides can also assist with changing bed linens and light housekeeping tasks to support a clean and comfortable environment.
Medical Director
The Hospice Medical Director is available 24 hours a day, 7 days a week to support your care and can serve as your attending physician, should you choose. Hospice becomes your first point of contact for urgent needs—replacing the traditional 911 response—with a nurse available to assess your condition and consult directly with the physician as necessary. Whether addressing issues such as falls, wound care, infections, or other medical concerns, our team is here to ensure your comfort, safety, and quality of life at every stage.
Volunteer Services
Volunteers are compassionate community members who offer support to patients and families when additional care or assistance is needed. They are available on an as-needed basis, depending on their availability, and the services they provide may vary.
